Thank you for your interest in this programme. Once completed, this form will help us with the selection and matching process.

Being mentored requires commitment from both the mentee and the mentor. Mentoring normally takes place during working hours and therefore you will need to seek permission from your organisation to take part in the mentoring programme where applicable – please do not apply to be a mentor unless you have the necessary permission.

Depending on the nature of topics the mentee wants to discuss, mentors may be required to have a separate confidentiality agreement between the mentee’s organisation and you.

The Chartered Institute of Fundraising, Canopi, 82 Tanner Street, London SE1 3GN is the data controller. 

Applications for the Mentor Programme will be accessed by members of the West Midlands committee.

The committee will use the information you provide to process your application and come to agreement about the outcome.

Application forms will be kept for one month after they are submitted - after which they will be securely destroyed.

We will only contact you by phone if we need to check on an aspect of your application or discuss an aspect of the Mentoring Programme with you if your application is successful. The number will be shared on a need to know basis for the purposes of the Mentoring Programme and will include sharing your number with your Mentee.
The email address you provide will be used for the purposes of the Mentoring Progamme and will be shared with your Mentee as well as those managing the Mentoring Programme should your application be successful.
